If you have employees covered by the Michigan UI law, you must register for an employer account. Register for most Michigan business taxes, and /or a sales tax license using this on-line e-Registration application.

This Form UIA 1718, Claimant’s Statement of Wages, must be completed by you if either you disagree with the wages reported by an employer or an employer did not report wages for you at all. After you file for benefits, you will receive Form UIA 1575C, Monetary Determination, which includes information such as your weekly benefit amount, the number of weeks you are allowed, and when to begin certifying.
