Form 11 - Declaration Form Every employee who is required or entitled to become a member of PF shall be asked at the earliest by the employer to furnish and shall, on such demand, furnish, for communication to the Commissioner, particulars concerning self and nominee.

Form 11 PF is a self-declaration form that contains the EPF history of the employee and is mandatory for an employee to fill out upon joining an organization. It can also be used to transfer the PF account automatically.

As the DSC of establishment are not registered with EPFO, the member has been informed to file physical claim (Form-13) for transfer of funds from his previous establishment.

Employee Provident Fund Organization (EPFO) has replaced UAN declaration form 11 with new EPF composite declaration form 11. Like previous UAN declaration form 11, now every employer needs to fill this form at the time of new employee joining.
