The Banner Health Employees 401(k) Plan (the “Plan”) was adopted as of January 1, 1974 and is intended to be a qualified retirement plan under Section 401(a) of the Internal Revenue Code. The purpose of the Plan is to assist eligible Employees in saving for retirement.
