For the restaurant you have to make a reservation, but not for the tour. We simply walked to the restaurant and asked to make a reservation for the evening.

Yes, Barão Fladgate is the Taylor Port restaurant on the Gaia side. This restaurant offers awesome views for lunch and dinner at moderate to high prices for the area. One of the best. Worth it if you can afford it and get reservations. The Grahms House is also nice.

We just turned up. No reservation required for the audio tour, but you need to phone in advance if you want a guided tour, they don't always have a guide available.
