If I book a ticket, will I have a time slot & if so will that ensure I get to go up at that time?

I don't think there's a set time slot system - it's limited to certain numbers at any one time on top, so the line steadily moves on, and we weren't rushed up Top either.
Is it possible to go up to the tower peak by stairs, not by elevator? If yes, do I need to pay for that?
